By participating in a Health Management Program, you’ll be continuously supported by a qualified clinician to guide you and answer questions that may be worrying you about your heart health. In coordination with your current doctors, we can help you make the best decisions for your health.

Healthy Weight for Life Essentials

Phone based program

This program is designed to help with chronic conditions and improve the overall health and wellbeing of eligible members by providing personalised support for weight management and nutrition. A team of specially trained dietitians will teach practical steps to achieve sustainable healthy living and weight management.

Program duration: 18 weeks

Healthy Weight for Life

At home program

This specialised cardiac management program helps eligible members better manage living with and reducing cardiovascular risk factors – all in the comfort of their own home. This program is suitable for those with a body mass index (BMI) greater than 28 and a cardiovascular risk factor (e.g. high blood pressure, high cholesterol, sleep apnoea) or a history of heart disease.

Program duration: 12 weeks

Programs to support your hospital admission

These programs can help you understand your hospital stay and get you back to your optimal health.

Hospital Support Program

Phone based program

This program gives eligible members access to an experienced registered nurse to help navigate the health system and understand their options before, during and after a hospital visit*. This program is in addition to an eligible member’s doctor’s guidance and advice. 

Program duration: Varies

*Not available for pregnancy-related hospital admissions.



Hospital

Cardihab

Phone based program

Cardihab is digital cardiac rehabilitation delivered via a smartphone app and telephonic coaching. The program supports eligible members who’ve recently been hospitalised for a heart-related event and aims to help them recover, get back to their optimal health and reduce the risk of future cardiac events. This program follows the Australian Heart Foundation and Australian Cardiac Rehabilitation Association guidelines and complements the care provided by your doctors.

Program duration: Six weeks

Advara HeartCare Heart Failure Clinics offer a broad network of specialised cardiologists and cardiac nurses to help manage heart failure with outpatient support at a participating clinic. This program is currently available in Victoria only.
