Healthy Weight for Life Essentials

Phone based program

This program is designed to help with chronic conditions and improve the overall health and wellbeing of eligible members by providing personalised support for weight management and nutrition. A team of specially trained dietitians will teach practical steps to achieve sustainable healthy living and weight management.

Program duration: 18 weeks

Healthy Weight for Life

At home program

This specialised knee and hip arthritis management program has been designed to help eligible members better manage joint pain and get back to living life – all in the comfort of you’re their own home. This program aims to improve daily living and quality of life by reducing knee pain, hip pain and stiffness, while also improving mobility with strengthening exercises and gentle physical activity. It can also improve an eligible member’s preparation for knee or hip replacement surgery, if relevant.

Program duration: 12 weeks

GLA:D Hip and Knee Osteoarthritis Management Program

This research-based program for people with hip or knee osteoarthritis utilises education and exercise which can help reduce symptoms associated with the condition. By reducing osteoarthritis symptoms such as pain, stiffness and mobility issues it can help to delay, or in some cases avoid the need for joint replacement surgery. If surgery is required, GLA:D® can also help with the preparation for knee or hip replacement surgery, improving recovery time and/or outcomes.

Program duration: 6-12 weeks

These programs and services can help eligible members understand their hospital stay or receive their hospital treatment from the comfort of home.

Hospital Support Program

Phone based program

This program gives eligible members access to an experienced registered nurse to help navigate the health system and understand their options before, during and after a hospital visit*. This program is in addition to an eligible member’s doctor’s guidance and advice. 

Program duration: Varies

*Not available for pregnancy-related hospital admissions.



Hospital Substitution

Orthopaedic Rehab at Home

Treatment at home

Orthopaedic Rehab at Home provides eligible members with the option to receive intensive physiotherapy in the comfort of their home where clinically appropriate* following a total hip or knee replacement to get them mobile as soon as possible.

Program duration: Varies

*The member's treating team will determine whether Orthopaedic Rehab at Home is suitable to facilitate an early discharge from hospital. This will occur where the same level of rehabilitation can be adequately provided in the comfort of the home.
