To be a recognised Healthier Lifestyle provider with nib, you'll need to complete an application form and have the appropriate qualifications for the area you wish to be recognised in. There are no costs involved in becoming a recognised provider with nib.
If a customer is claiming for a gym membership or personal trainer services, a Health Management Program form needs to be completed by the treating practitioner recommending the service. For any other providers in the Wellness category, the customer will need to follow their standard claiming process procedure. You will need to complete a new registration form if you change address. If you have multiple business locations, a form for each location will also need to be completed.
