About this organisation

The NSW Outback Division of General Practice is a not-for-profit organisation that plays a key role in providing culturally safe and accessible primary health services to rural and remote communities across Western NSW. Their focus is on improving healthcare integration, ensuring better access, and promoting equity in healthcare for individuals and communities in the region. 

Bourke Pathways

The issue

nib and NSW Outback Division of General Practice developed Bourke Pathways based on consultation with local service providers and community groups. The care navigation program is designed to support residents of Bourke Shire in accessing existing health, community, and social services. The aim of this pilot program is to improve health outcomes and support self management and health literacy.  

The program works alongside local residents, service providers and community groups to create a new support system, from assistance with referrals, paperwork, or simply knowing where to turn for help. Locally based staff will help guide clients to timely access to care and foster self‐management and independence through education and support, while addressing obstacles to better health

Why it matters

Many people in rural communities like Bourke struggle to find the right services or feel overwhelmed by the paperwork involved. Geographic isolation often means long distances to medical facilities with few public transport options, compounded by a shortage of healthcare and other service providers and limited availability of specialised services.  

With many community members unaware of available services or unsure how to access them, the Bourke Pathways care navigation program will bridge the gap by guiding individuals through the process, offering personalised support and referrals. The program is dedicated to simplifying these processes and ensuring that all eligible clients can get the help they need, when they need it.
